Digital Form and Process Change Request

Please use this form for any changes to existing digital forms or processes. If the form is not in Etrieve or TeamDynamix yet, please complete a Digitization Request.

Changes can include

  • Adding or removing form fields, for example 'Add an additional field so students can indicate which campus they are studying at'.
  • Adding or removing form functionality, for example 'Automatically fill in the student's address so they don't have to enter it manually'
  • Changing the workflow, for example 'We need to form to go to Financial Services after it's been approved by the Registrar's Office'.
  • Modifying who can access a form, for example 'The form will now be used by all staff, not just Faculty, so all staff will have to be able to see it.'

 

When you should NOT use this form

  • Something on a form is not working. For example, if you receive an error message when trying to submit a form and can't see why, please submit a Service Request.
  • To provide general feedback for a form, if you are not the form owner. To submit feedback, please use the Etrieve Feedback form.
